What is personal information?
Response: Personal information is any information that identifies you, or by which your identity can be determined.
What information about a user is collected on the website and where is it collected?
Response: Your personal information is collected only by lawful and fair means and not in an unreasonable or intrusive way. Wherever possible it is collected directly from you, when you sign up to be a member of VitalizeU or in the process of your participating in your membership. Your name, address, email, phone numbers, and information about your health and progress, as entered by you, may be collected.
We may use your information to send you a welcoming email or to confirm your user name and password. If you request, we may also send you electronic newsletters and information about products and services that may be of interest to you. If you no longer wish to receive these communications, you have the option to change your preferences and request that you be deleted from the communication list.
Information that you offer in our chat rooms or bulletin boards, for discussions and idea sharing between members, is public and will be collected and stored. Any personal questions you ask from a coach or consultant are also collected and are not shared with other members, unless you choose. Private questions and answers that are made public become part of the frequently asked questions (FAQ) and answers public database. If you send us an email with questions or comments, VitalizeU may use your information to respond to your questions or comments. VitalizeU reserves the right to add any questions or answers to the general FAQ database available to the public, however in this event, personally identifying information will be deleted, and words may be revised to protect your privacy.
Coaches and other consultants may only use the information you provide to respond to you, and are not entitled to use the information for any purpose other than responding specifically to your questions and assisting you with your goals related to VitalizeU.
The site includes online purchasing, so the history of any purchases you make will be tracked. We track the pages visited by members however we do not leave cookies.
Are you going to be collecting information from children under 13 years of age?
Response: We do not knowingly collect information from children, or individuals under the age of 18. Our membership comes from corporations, and we have specified to the companies that children may not be included in the program.
What do you do with the information collected?
Response: The information will be used primarily for VitalizeU and to ensure our members are able to obtain results from their membership. Members have the ability to sign up for or opt out of newsletters, promotional information and emails. All correspondence with the member is done via email, or via website information.
What choices does a person have regarding the collection, disclosure and distribution of personally identifiable information?
Response: Each member has the ability to show their identity on the site, or create an alias user name to be used when publishing questions and comments on the chat rooms. In addition, ghost email addresses are used. Members may hide their identity entirely from other members, coaches and other consultants on the site.
Do you transfer any personal information to an employer?
Response: No, we do not disclose personal information to your employer. We do, however, provide feedback to companies about their employees’ progress on the system. Individual information will not be shared, but trends or summaries will be disclosed. Benefits of the program, such as sick days and vacation days, will also be calculated based on other statistical information from each company, if requested by the human resources department.
When will personal information be disclosed?
Response: Your personal information is secure and will not be disclosed. The information is for the use of VitalizeU and its members, only. The member list will never be sold. If you receive any email communications, such as a promotional package that is offered with the approval of your company, it is sent through our secure database. An example of this would be an introduction package with login and site help information.
The information collected will not be given to third parties. Credit cards are an exception, since we use a credit card processing service for e-sales, and are therefore required to pass the credit card information to the processing company. This transfer is done using secured encrypted transition. The processing company is bound by law to protect your personal information. Credit card information (other than last 4 digits, and authorization number) is not stored in the VitalizeU database. VitalizeU stores only the minimum amount of information that is required for audit and accounting purposes.
Other exceptions may be where the information is required to be disclosed by law, such as where a court issues an order or a subpoena or where you have consented to the disclosure such as chat rooms, and public ask-the-coach (FAQ) questions that are available to other members using the site.
What security precautions are being taken for the information collected?
Response: VitalizeU takes all reasonable precautions to ensure that your personal information is kept safe from loss, unauthorized access, modification or disclosure. Members sign into the site with a unique username and password. You have the option to change your password at any time. Critical data, like passwords and payment information is stored in secure encryption in the database. This means we have put technological safeguards in place to prevent unauthorized computer access to the information.
How long does VitalizeU retain the information?
Response: VitalizeU will retain the information for as long as you are an active member and for a period of time after that, as required by applicable law.
Is there a link to update or correct personally identifiable information?
Response: It is important that your information is accurate and up-to-date. Once a member has logged into the site, the member has the ability to set or change your information, and preferences for use throughout the site. If any of your information changes during the course of your membership, the necessary changes can be made. Changes are dynamic, so if a person wants to hide their identity, it will be hidden for all transactions, including chat rooms, present, past, and future.
Can you be denied access to your personal information?
Access to your personal information may be denied in some circumstances, such as for example when it would have an unreasonable impact on other people's privacy. We will always explain why.
